Guest Player Forms (By States):
For Virginia Players:
- Permission for VA/DC Residents to play on a Maryland Team
- Permission for VA/DC Residents to play in another State, other than MD
- Permission for a VA Player or VA Team to play in another State to play in another State
- Interstate Region III Form for Players wanting to play in North Carolina & Tennessee
- Interstate Guest Player Form (A player to guest with a Team outside of VA or a player from outside of VA to guest with a VA team)
For Maryland Players:
- Permission for a Maryland Player to play on a VA/DC Team to play on a VA/DC Team
- Permission for Maryland Residents to play on a Virginia Team

Permission to Travel:
- Permission to Travel to a US Youth Soccer Sanctioned Event (On-Line Permission)
- Permission to Participate in Another Organizations Sanctioned Event
(Events outside of US Youth Soccer - ex: US Club, Super Y)
International Paperwork for Travel:
Submit the following paperwork to the VYSA State Office for International Travel. Once VYSA approval is granted, the applications will be forward to US Soccer:
- A completed Application for Foreign Travel (must be the original no copies or faxes)
- A copy of the official brochure, pamphlet, invitation or applicable material for the Tournament.
- A copy of the approved roster which includes all players and team officials
- A $50 check per team payable to US Soccer
- A signed Ted Stevens Olympic and Amateur Sports Act signed
- VYSA On-line Permission to travel
Forms to Host a Tournament involving Foreign Teams:
Submit the following paperwork to the VYSA State Office 45 days prior to the tournament. Once approval is granted by VYSA, the applications will be forward to US Soccer:
- Checklist to assist in submitting Paperwork to Host Foreign Teams
- Application to Host A Tournament or Games Involving Foreign Teams
- Hosting Agreement for a Tournament or Games Involving Foreign Teams
- Ted Stevens Olympic and Amateur Sports Act
- A check for $75.00 (non-refundable) payable to US Soccer
